Online Forms For Douglas Family Eyecare

As of November 18, 2024, the Optomap® is a required testing for all comprehensive exams at Douglas Family Eyecare. Unfortunately, at this time, Optomap® is not covered by insurance. The new cost of this test is $25, reduced from the already discounted rate of $35.

We pride ourselves on providing our patients with the best possible standard of care. The Doctors strongly believe that the Optomap® Retinal Exam is an essential part of your comprehensive eye exam and prescribe it for all patients once per year. It is an important tool used to manage effects of high blood pressure, diabetes, retinal tears, moles, and other conditions in the back of the eye, and can better help detect these conditions earlier and any changes year to year. It is our belief that an eye exam without this element is incomplete.
